Are you using version 7.01 if so most likely you might have this problem.
Couple of things to check.
When you double click on the item, you can see various item details screen.
In there check out for document flow tab, if it has an entry for putaway planned (then the task has been created, but it was unable to save it to db in the process the status is messed up)
Seconldy check the Status tab, there you should see a status DEU and it should be set to 1.
In all fairness if it is a test delivery and none of the above matches just recreate the delivery.
If it happens always you can raise it with SAP.
